Academic Transcripts

In compliance with the Family Educational Rights and Privacy Act of 1974, transcripts may be issued only at the written request of the student. Parents may request a student’s transcripts only if they can demonstrate with federal income tax documents that the student was their legal dependent for the most recent tax year. No transcript will be issued for any student or former student whose financial obligations to Prescott College have not been satisfied.

Types of Transcripts
All types of transcripts require a signed request.

  • Official Transcripts
    A Full Transcript will include: Cover page with grades/credits and degree(s), official Prescott College seal, transcript key describing the Prescott College academic system, course descriptions, and narrative evaluations (if requested). An official Summary Transcript (cover page only) may be sent when a very short deadline precludes compiling a student's narrative evaluations.

  • Unofficial Transcript
    Cover page with grades/credits and degree(s) only.

Cost of Transcripts
There is no charge for Prescott College transcripts.

Requesting Transcripts
Requests must be made in writing to the Office of the Registrar 2-3 weeks in advance of the date needed. All requests must be submitted using the Transcript Request Form (No Exceptions to this policy will be made).  Because of the need for the student’s signature, we cannot accept phone or e-mail requests.

You can download a Transcript Request Form in  Word Logo Word or  Acrobat Logo PDF format.  Please print, sign and fax or mail this form.  You may also email a scanned copy that contains your signature. 

Please Note: Prescott College can only be responsible for timely delivery of transcripts when information provided by the student is accurate and complete.  Transcript requests that are not signed or are missing information cannot be processed. 

*Online Format: Depending on your computer's configuration, there may be 2 pages to this form. Both must be included in your request.

Faxing Transcripts
A faxed copy of a transcript is automatically an unofficial transcript. We only fax summary transcripts (cover pages, no narratives). Students are responsible for supplying a correct, working fax number.

Please Note: A transcript is sent directly to the school, firm, agency, or individual designated by the student's request. It is sent in a sealed envelope and should not be opened by the student. Most institutions will not accept opened or unofficial transcripts.

Copies of transcripts from other schools cannot be sent by Prescott College. Students must request transcript copies directly from that school.
