Diagnostic Essay Letter
All students are required to complete the diagnostic writing essay.  Prescott based students complete this prior to or during Orientation and Tucson students complete this as part of the Adult Liberal Learning (ALL) course. The diagnostic essay is different than the goals essay that was written as part of the application process. It requires students to assemble their thoughts in a limited time under proctored conditions.

Pre-contract Worksheet
This form is completed by the student to brainstorm and communicate ideas for the course. Once completed, it should be given to the mentor prior to or during the first meeting for feedback.

Study Contract Form
This is the official form which documents the course and initiates payment to the mentor. The student completes a draft contract at the beginning of the course and then the mentor provides feedback to be incorporated in the final contract. Both the student and mentor sign the contract and send it to the student's core faculty for final approval. The core faculty may send it back for revisions. The study contract should be submitted within the first 3 weeks of the course.

Evaluation Form
This form is completed by both student and mentor at the end of the course. The student completes the upper two-thirds. The Course Description is written into the first blank. The student also completes and signs the Student Evaluation section. The Student Evaluation should be the student's statement of what s/he did, learned, encountered, appreciated most, had most difficulty with, etc. Any reader should be able to gain a clear sense of what this course actually contained for this student. A length of 100 to 150 words is best (maximum of 150 words). The mentor should read the Student Evaluation before writing the Mentor Evaluation and can suggest revisions if s/he thinks it advisable.

The mentor completes the Mentor Evaluation section, taking into account the amount of credits requested, whether the course is to be transcripted as upper- or lower-division credit, and whether or not a letter grade is requested. The Mentor Evaluation can refer to the Student Evaluation and then go beyond. As with the Student Evaluation, the Mentor Evaluation should communicate what the student studied, what s/he did, what s/he learned, what went best, and may comment on areas of weakness or concern, or make recommendations for further study. Again, 100 to 150 words are an appropriate length (maximum of 150 words).

The entire evaluation becomes part of the student's permanent transcript. Each section should be typed, thoroughly written, and carefully proofread. The student should keep a copy for his/her records. It is the student's responsibility to make sure the mentor submits the completed Evaluation Form to the ADP office. Credit cannot be given without completed Evaluation Forms.

Student Evaluation of the Mentor
This form is completed by the student for the purpose of evaluating and providing feedback to the mentor. The completed forms are kept in the ADP office. Mentors can request copies of the evaluations by contacting Jody Lichtenberg, (Prescott students) or Jane Ryder (Tucson students).

Research Paper Approval Form
All degree seeking students must have a research paper approved at least 3 months prior to the intended graduation date. The paper is completed within a mentored course and must be approved by the mentor, core faculty, and ADP writing specialist.

Teacher Certification Request Form
The student who has completed all required courses for teacher certification completes this form. It is submitted to the College’s Certification Officer in the Registrar’s Office.

Leave or Withdraw Request Form
The ADP Leave or Withdraw Request form is to be used when an ADP student decides to take a Leave of Absence (for three or six months), withdraw from Prescott College, or has finished his/her program of study and does not need to re-enroll. This form should be faxed or mailed to the Office of the Registrar and will be acknowledged when received.

Curriculum Documentation Format
Students are requested to use this Curriculum Documentation (CD) template to ensure their CD is in the proper format for review by the Curriculum Committee.

Application for Graduation
This form is completed by the student and submitted to the core faculty member. The form must be completed and attached to the Curriculum Documentation at the time of its submission (at least 6 months prior to the intended graduation date).

Mentor Forms
Mentor Interview Protocol
This form is completed by the mentor prior to or during the first meeting with the student, after the student has turned in the Pre-contract Worksheet. The purpose is for the student and mentor to communicate their ideas and expectations before constructing a study contract.

The Mentor Packet provides an overview of the Adult Degree Program mentoring process and specific information about the mentor responsiblities and the stipend process. This document also provides introduction to the various paperwork that ADP mentors may encounter through their work with students.

W-9 Form
The IRS requires that the W-9 form be completed by the mentor and submitted to the College. Payment of stipends cannot be made without a signed W-9 on file.
